¿Qué es la osteopatía y en que casos se recomienda como tratamiento?
La Osteopatía es un sistema de medicina manual cuyo objetivo es restaurar la salud y el bienestar. Es un tratamiento de salud integral basado en manipulaciones de la estructura del cuerpo (articulaciones) y de los tejidos, que tiene como efecto un reequilibrio en la fisiología del cuerpo con una repercusión en todos sus sistemas (por ejemplo linfático o digestivo). Está recomendada para personas de todas las edades, al igual que la medicina alopática.
¿Se clasifica la osteopatía como medicina alternativa?
La Medicina tradicional o alopática trata las patologías con fármacos y cirugía. La Osteopatía se encuentra en el grupo CAM (siglas inglesas de Medicina Alternativa y Complementaria), no prescribe drogas y trata las disfunciones y problemas de salud a través de una terapia manual del cuerpo acorde con los estudios científicos de anatomía y fisiología. Actualmente se sigue llevando a cabo un gran esfuerzo en investigación osteopática con base científica a nivel mundial, tanto de pre-grado como de post-grado.
Recientemente la Osteopatía también ha pasado a ser denominada como Medicina Integrativa, término que hace referencia a la utilización de medicina tradicional y alternativa en el mismo tratamiento. (Un ejemplo cercano de ello es la utilización de osteopatía en la unidad pediátrica del Nuevo Hospital de Mataró)
¿Cuáles son los principios básicos de la osteopatía?
La Osteopatía está basada en el estudio de la anatomía y la fisiología, su relación, su interacción e interdependencia. Presenta una visión del cuerpo unitaria.
La Osteopatía busca la causa de los síntomas y trata esta causa, eliminando los obstáculos (congestiones, contracciones, etc.) que no permiten al cuerpo autorregularse y recuperar la salud. La causa puede ser interna o externa (dieta, relaciones sociales insatisfactorias, problemas laborales, malas posturas…)
Principios básicos de la osteopatía:
La regla de la arteria es suprema: el flujo sin impedimento de todos los fluidos corporales denota una buena salud. Congestión o estancación de cualquiera de estos líquidos llevará a una toxicidad (a nivel metabólico) y enfermedad/disfunción consecuente. La mejoría en la circulación de la sangre, linfa o LCR (liquido cefálico raquídeo) proveerá de nutrientes y eliminará las toxinas (metabólicas).
La estructura gobierna la función: se entiende “estructura” como la estructura del musculo esquelético, y “función” como la fisiología. Hay una relación entre ambas que es bidireccional e interdependiente. Las alteraciones en la estructura del cuerpo llevarán a defectos funcionales y viceversa.
El cuerpo es una unidad: la restricción de movimiento en una articulación, o la presencia de una contractura o una congestión, se reflejarán en otras partes del cuerpo y no necesariamente en el lugar de la disfunción o dolor. Todas las partes del cuerpo están interrelacionadas e interconectadas.
La Osteopatía postula la autorregulación del cuerpo: el cuerpo tiene mecanismos intrínsecos para la curación y autorregulación. Tiene todas las sustancias químicas necesarias para promover y mantener el estado saludable. Se cree que las lesiones crónicas ocurren cuando la capacidad de autocuración está comprometida.
Otros Principios:
Una buena postura indica una buena salud funcional. Las recomendaciones para cambiar hábitos pueden incluir desde cambiar las almohadas a utilizar una silla ergonómica, pasando por cambiar de dieta alimentaria, hacer más ejercicio, incrementar la ingesta de líquidos... todo lo cual también forma parte de un tratamiento osteopático.
¿Cuáles son las técnicas principales del tratamiento osteopático?
Las técnicas principales incluyen estiramientos y movimientos suaves de las diferentes articulaciones del cuerpo, y son:
- Masaje del tejido blando superficial y profundo.
- Drenaje linfático. Inhibición de puntos específicos (presión contra un punto muscular).
- Sacro Craneal: escucha del ritmo inherente del cuerpo.
- Visceral: masaje suave y movilización de los órganos internos y sus ligamentos. HVT (Thrust) movimiento corto y brusco indoloro para ajustar un bloque articular.
- Fascial: localización y regulación de las tensiones fasciales del cuerpo (la fascia es una estructura fina membranosa que cubre todos las estructuras corporales formando una red de conectividad).
- TGO: (tratamiento general osteopático) consiste en movimientos suaves, rítmicos y rotatorios de todas las articulaciones del cuerpo. Es muy relajante y entre muchas otras ventajas es excelente para el drenaje linfático y la realineación de la postura corporal.
Existen todavía muchas más técnicas. Cada paciente, según su edad o tipo corporal, recibe el tratamiento con la técnica más indicada en aquel momento para dicho paciente.
¿Qué tipos de disfunciones pueden ser mejor tratadas con Osteopatía?
No solo los dolores de espalda, cervicales o lesiones deportivas, sino también asma, estreñimiento, diarrea, acidez, ciática, dolores de cabeza, alteraciones menstruales, posturas, problemas de rodilla, tendinitis, bruxismo, ATM, dificultades de atención y concentración, baja energía e insomnio. Es una lista sin fin. Trata casi todas las disfunciones, dejando aparte por ejemplo infecciones (aunque puede tratar algunos de los síntomas presentes).
¿Existe una entidad reguladora para la osteopatía en España?
Sí, se llama el Registro de Osteópatas de España (ROE). Sólo se permite a ciertas escuelas reconocidas asociarse, de modo que ser miembro del registro confiere una cierta garantía de calidad en la formación, y además garantiza un seguro de trabajo. Soy miembro del ROE y mi escuela está afiliada a la prestigiosa European School of Osteopathy en el Reino Unido, donde hice mis exámenes y recibí mi certificado.
¿Puede la osteopatía complementar el tratamiento médico?
Por supuesto. Si tiene dolor de espalda porque hay tensión debido a una mala postura, el tratamiento osteopático se dirige a cambiar su postura y reeducar los tejidos de forma que la postura correcta pueda mantenerse. Si tiene asma crónica, esto causará un estrés mecánico en todos los músculos respiratorios y en el movimiento torácico, lo cual generará un estado de tensión y restricción, y también afectará los sistemas simpático y parasimpático que regulan las contracciones y dilataciones de los bronquiolos; la osteopatía puede aliviar estas tensiones y reequilibrar los sistemas. Si tiene el sistema digestivo lento o acidez, las sesiones de osteopatía pueden ser una opción más valida y duradera que el consumo de pastillas de fibra y antiácidos, sin tener los efectos segundarios de estos. Si padece de dolores de cabeza constantes con un uso constante de calmantes, la Osteopatía busca la causa del dolor y lo elimina reduciendo así el uso de dichos fármacos.
¿Puede un osteópata aconsejar sobre asuntos ambientales o de estilo de vida que están contribuyendo a generar una disfunción?
Sí, ciertamente. Al paciente se le hace una anamnesis completa en su primera visita, y si se considera que el origen del problema es orgánico o necesita pruebas médicas se derivará al especialista. Si el origen de la causa es la dieta se derivará a un especialista en dietética. A menudo se propondrán como parte del tratamiento ejercicios específicos y cambios en el estilo de vida. Malas posturas y tendinitis de muñeca/codo se mejoran con osteopatía, pero hay una reincidencia si no se cambia la postura cuando se está sentado, o se pasa mucho tiempo en la silla de trabajo.
¿Cuáles son los beneficios de visitar un osteópata durante el embarazo?
Estando embarazada se altera la postura corporal (el centro de gravedad se adelanta), y el cuerpo se somete a múltiples cambios hormonales. Estos cambios posturales pueden causar tensiones musculares, dolores lumbares y piernas pesadas. Pueden hincharse las piernas, que mejorarán con un drenaje linfático. Dolores en el suelo pélvico o sacro pueden ser tratados y liberados, relajando y aliviando el dolor.
¿Osteopatía en cuidados post-natales?
Se puede dirigir el tratamiento al bebé (ver abajo) o a la madre. La osteopatía puede ayudar a reequilibrar los sistemas que controlan los grandes cambios hormonales que ocurren durante y después del embarazo. Puede mejorar la postura, aliviar las tensiones, contracturas y dolores generados por malas posturas al llevar a los pequeños en brazos, o aliviar el dolor creado por posiciones no comunes cuando se amamanta. Puede trabajar el suelo pélvico para ayudar con problemas de hemorroides o incontinencia leve, liberar contracturas de los músculos de la zona sacro glúteo, todo lo cual reducirá tensiones y eliminará dolor en las relaciones sexuales. Un tratamiento general osteopático ayudará a mover el fluido linfático y reducir la congestión que ha provocado las piernas cansadas o hinchadas.
A menudo se recomienda osteopatía craneal para los bebes, ¿por qué?
Cuando el bebé pasa por el canal del parto, esto le puede causar tensiones en las membranas craneales que hay debajo de los huesos del cráneo. Estas son muy maleables en el neonato y las suturas todavía no se han formado completamente. Liberar tensiones a una edad temprana puede ahorrar muchos problemas de salud en el futuro. Los bebes responden muy bien a estos tratamientos, los cuales son muy suaves, ligeros y 100% indoloros.
¿Qué problemas se pueden resolver en niños de más edad?
Problemas de cambios de humor. Los tratamientos pueden ser tranquilizantes, ayudar al niño a relajarse y consecuentemente conseguir que duerma mejor. Los niños que crecen rápido o tienen sobrepeso pueden tener problemas para mantener una buena postura, y la osteopatía puede ayudarles a mejorar. Tendencia a sufrir ataques de pánico, diarreas, estreñimiento: su sistema nervioso puede regularse y conseguir a largo plazo fortalecer su sistema inmunitario. Infecciones de oído y congestiones pueden ser tratadas. Los problemas de concentración pueden ser debidos a tensiones craneales, contracturas, problemas de visión, dieta… todo ello puede ser diagnosticado con el tratamiento.
¿Cuánto tiempo recomienda que la gente continúe visitando a su osteópata para el tratamiento de una disfunción?
Depende de muchos factores: el tipo de lesión que presenta, si es crónica, aguda, la edad de la persona, cómo responde al tratamiento, su estado de salud en general.
En un caso agudo después de la primera visita se hará la siguiente sesión al cabo de 7-10 días, después a las 2 semanas, y más adelante entre 4 y 6 semanas. Una vez dada la persona de alta, ésta puede volver a visitarse cada 2-4 meses, para un seguimiento general. La media de los tratamientos es de 3 a 5 sesiones.

What is Osteopathy and who does it target?
Osteopathy is a comprehensive approach to health. It is a system of manual medicine aimed at improving health and well-being. Minor adjustments are carried out on the body’s structure (bones) and the tissue (muscle) and this influences or rebalances the physiology of the body`s various systems (for example digestive, respiratory or lymphatic). It targets all groups of people in the same way as traditional medicine; newborn, toddlers, children, teenagers, adults and the elderly.
Is Osteopathy regarded as an Alternative Medicine?
Osteopathy falls in a group called CAM (Complementary and Alternative Medicine) as it does not prescribe drugs (as does Allopathic medicine), it treats dysfunction and pain with the hands, carrying out manual treatments on the body.
There is a concerted movement to increase the amount of published Osteopathic evidence based research in many parts of the world, to align it with Allopathic medicine.
There is a new term in current use called Integrative medicine. This is the use of alternative medicine and traditional medicine in the same treatment. This is the future of medicine (An example is the use of Osteopathy in the Paediatrics unit in the new Mataro hospital).
What are the main principles of Osteopathy?
Osteopathy is based on the study of anatomy and physiology and their relationship, interaction, and interdependence. It looks at the body as a whole.
It looks for the cause of the problem and attends this cause, eliminating the obstacles that are not allowing the body to make itself healthy (congestion, contracture etc). The cause may be external; diet, poor work/social relationships, posture.
The main principles of Osteopathy are outlined below:
The artery is supreme; unimpeded flow of all body fluids denotes good health. Congestion or stagnation of any of these fluids leads to toxicity and thence illness/dysfunction. Improved circulation of blood, lymph, or CRF (brain/spinal fluid) will provide nutrients and eliminate toxin build up.
Structure governs function; structure is taken to denote the skeletal structure (musculoskeletal) and function (physiology). There is an interdependent and bidirectional relationship of these two. An alteration in the body’s structure leads to functional defects and vice versa.
The body is an integrated unit; restriction of movement in a joint/articulation, contracture or congestion will be reflected in other parts of the body, and not necessarily at the site of the dysfunction or pain. All body parts are interconnected and interrelated.
The body is a self-healing unit; the body has intrinsic mechanisms for self-regulation and healing. It has all the necessary chemicals, substances to promote and maintain health. Chronic conditions are believed to occur when the healing capacity is compromised.
Extra principles:
Good posture indicates good functional health. Recommendations for lifestyle changes may include changing pillows, buying ergonomic chairs, taking more exercise, dietary change or increased water intake, all form part of an osteopathic treatment.
What are the main techniques of Osteopathic treatment?
The main techniques involve gentle stretching and moving of different articulations.
Soft and Deep tissue massage.
Lymphatic drainage. Inhibition of specific points (pressing the hands on a muscle).
Sacro cranial: listening to the body’s inherent rhythm.
Visceral; soft kneading or stretching of organs; stomach, intestines.
HVT(Thrust); short thrust to move a misaligned vertebra or joint.
Fascial; listening and moving with the body’s fascial structure (a thin membrane that covers all the body’s
structures; organs, muscles that hence forms a network of connectivity),
General osteopathic treatment is like a rhythmic massage; mobilizing all articulations in the body, it is excellent for drainage and realigning body posture.
Diaphragm /thorax pump for asthma.
There are many more techniques. Each patient, for age or body type, is treated with the most suitable technique at that time for that specific person.
What type of disorders can best be treated by Osteopathic treatment?
It is not only back and neck aches, or sports injuries, but includes asthma, constipation, diahorrea, heartburn, sciatica, lumbago, headaches, menstrual problems, posture, knee problems, tendonitis, bruxism, TMJ problems, neck pain, attention difficulties, poor concentration, lack of energy, insomnia. The list is endless. It treats most dysfunctions. It doesn’t, for example, treat infection or disease (though is able to treat some of the presenting symptoms)
Is there are a regulatory body for Osteopathy in Spain?
Yes there is, it is called the Registro de Osteopatas en España (ROE). Only recognized schools are allowed to join. Being a member confers the guarantee of your standard of schooling and insures you. I am a member and my school is affiliated with the prestigious European School of Osteopathy, UK, which is where I took my exams and received my certificate.
Can Osteopathic complement advice from Doctors in some cases?
Absolutely! If you have bad back ache because you are straining your lumbar spine because you are not standing correctly, an osteopathic treatment will be directed at correcting your posture and putting your body in a state where this will be maintained. If you have chronic asthma, this will have caused mechanical strains on the muscles involved in breathing and thoracic movement, also affecting the Sympathetic and Parasympathetic systems involved in vasodilatation or constriction of the bronchioles; osteopathy can relieve these tensions and rebalance the systems. If you have a chronic slow digestive system or heartburn…osteopathy may be a valuable and longer lasting option than heartburn/antacid/ fibre pills. Constant headaches and constant painkillers; we look at the cause of the headache and take this away thereby reducing or eliminating the use of NSAIDS.
Can an Osteopath also advise on lifestyle and environmental issues that may contribute to a disorder?
Yes, definitely. The patient gives an in-depth medical history on the first visit, and should it be deemed that the origin of the problem is organic they will be referred to a specialist. Or if the emotional /psychological side is the origin is the cause they will be referred to the adequate specialist. If diet is the cause, they will be referred to a dietician. Specific exercises and lifestyle changes are often suggested in consultation. Poor posture and tendonitis of the elbow/wrist will be improved in Osteopathy but will reoccur if the sitting posture/chair does not change.
What are the benefits of visiting an Osteopath during pregnancy?
Being pregnant alters the body posture (your centre of gravity has moved forward), along with a lot of hormonal changes. These postural changes can strain different muscles and you may find yourself with low back ache, or in the feet/legs. You may find you have swollen ankles; this will benefit from a drainage treatment. Sore pelvic muscles or sacrum can be treated and released giving relief and relaxation from pain.
What about Osteopathy & post-natal care?
It can be addressed to the baby (see below) or to the Mum. Osteopathy can help rebalance the systems that are stimulating and controlling the great hormonal changes that occur during and after pregnancy, improve posture, relieve tensions and contractures from carrying small but heavy children around, or relieve pain created from unaccustomed positions during breast feeding. It can work on the pelvic floor to help incontinence /haemorrhoid problems, release contractures in the sacrum area/gluteal muscles, all of which will reduce tensions and eliminate any pain suffered in sexual relations. A General Osteopathic Treatment will help move lymph fluid and reduce congestion that that has led to heavy or bloated legs.
Cranial Osteopathy is often recommended for babies, why is this?
When the baby passes through the birth canal, there may be tensions created in the cranial membranes, under the bones in the skull. These are very pliable in new-borns as sutures are, as yet, not fully formed. Babies respond very well to these treatments which are exceptionally light touch treatments and 100% non-painful.
What problems can Cranial Osteopathy help with in older children?
Problems with mood changes; some treatments can be very calming and may help the child/teenager relax, and consequently sleep better. Taller or overweight children may be struggling with good posture and treatment will help improve this. Tendency to panic attacks, diahorrea, and constipation: the digestive system can be regulated and gives a long-term net effect of a stronger immune system. Ear infections and congestion. Problems of concentration may be due to tensions, strains, contractures, poor eyesight, diet, all this can be assessed in the treatment.
How long do you suggest people continue to visit an Osteopath when treating a disorder?
It depends entirely on the presenting lesion of the person, how long they have had it for, and how well they respond to treatment, also their general state of health and well-being. In an acute case the first visit would be followed up after 7-10 days, then 2 weeks, then 3 weeks, and then once every 2-4 months if the person wants to have a “maintenance” session. Average treatment about 3-5 sessions. If there is no improvement, they will generally be referred to the appropriate specialist.
